Instructions for Requesting a Loan:
1. Download the request form. It’s page 3 of the Purchase and Loan of Educational Materials policy.
2. Complete the form and mail it to Linda Menard with a check for $5 made out to her. Her address is TAMC, PO Box 151, Presque Isle, ME 04769. The form can also be emailed to her at lmenard@tamc.org with a note saying check to follow.
3. Call Linda at 207-768-4172 with any questions.
